As we near the first day of practice, be sure you have completed the physical, concussion form, and insurance waiver. If you go to USD231.com, then select Trail Ridge Middle School, at the top in the middle of the page is a tab that says ACTIVITIES&ATHLETICS. Hover over this drop down menu and you will see "participation and required forms", select this, then you can download ALL three of the forms you need to have completed. Once again, these all three must be filled out and signed in their entirety if you wish to participate on the first day of practice! We look forward to seeing you on day 1!
